I think ZB is an awesome tool for morphing, sculpting and adding details but I wouldn't use it as my primary modeling tool. Extracting parts of the figure's mesh is a no-go anyway for commercial products.
Google for tutorials on box modeling, that will show you the procedure most PAs use for shoes. With ZModeler you could do that even in ZB.
I have no idea where you can make them as I have no idea what software you have skills in.
Any 3D package that allows for the creation of 3D objects, and their export as .obj files, will do. Simply find the one that suits your style.
Start with those that are free, as throwing cash at the problem will use cash, and not solve the problem.
... Oh, and identify the problem.
I almost always use Blender; I occasionally try and learn a bit more of Zbrush, but it is slow going for me: Blender I found easy, when I started years ago.
Being a bit blunt, if you're relying on extruding and modifying the base mesh, that's only the absolutely most basic method of making clothing (and one that as esha says, is a no-go for any commercial purpose).
You need to learn the creation of meshes from scratch. "Box modelling" is indeed one place to start looking, but I'd also suggest taking a look at "edge modelling", which I personally tend to prefer. They've each got their advantages, so it's a matter of which you personally take to.
The basics of modeling the shoes should be the same across modeling applications, with the details of how to achieve those basics varying based on the program you use. I don't begin to claim any sort of expertise, though. My modeling skills are less than minimal. Mostly I use Hexagon to create a morph here and there, and when I don't use it for a while, I'm lost all over again when I open Hex.
Here is a link to the search results on Youtube for modeling 3d high heel shoes. Most of these are for various 3d programs, but a few "how to walk" in heels videos showed up, too. Maybe the program you're using will be in there, but if not, having some experience with your modeling program creating clothing, you should be able to follow along and apply the concepts with the tools you're used to.
Once you have modeled the shoes, you'll need to rig them. I came across a video a couple of years ago, on rigging shoes for G3F. I learned a lot from it, trying to convert G3F shoes to G8F. Results vary, with the heel being the biggest pain, as Daz removed the Heel Bone from G8. But I think you'll still find it helpful: Rigging High Heeled Shoes in DAZ Studio for Genesis 3.
Good luck. I hope you'll share some renders of your successes!
THANKS YALL!.
